Intuitive Surgical stock tumbled 10.9% in pre-open trading to reach $358.50 after the robotic surgery leader delivered a Q2 2026 earnings beat that nonetheless failed to satisfy investors focused on slowing U.S. procedure growth and an unchanged full-year outlook.
The company posted stronger-than-expected results, with adjusted earnings of $2.80 per share on revenue of $2.89 billion, compared with analyst forecasts of $2.51 per share and $2.82 billion, but the stock fell sharply after hours as investors focused on signs of slowing U.S. procedure growth and a cautious outlook. U.S. procedure growth slowed to 12%, down from 14% in the first quarter, as management cited ACA subsidy changes and deferred procedures as contributing factors.
Intuitive Surgical left its full-year 2026 da Vinci procedure growth forecast unchanged at 13.5% to 15.5%, and said it still expects growth to come in near the midpoint of that range, noting that the first half of the year ran closer to 15% but that some moderation is expected in the second half due to tougher comparisons and seasonal effects.

The broader market offered no relief, with the NASDAQ declining 1.8%, the S&P 500 falling 0.9%, and the Dow Jones slipping 0.7%, creating an additional headwind for high-multiple growth names.
At the heart of the slide is valuation compression — Intuitive Surgical trades at a price-to-earnings ratio near 49, well above its medtech peers, and that premium multiple has been bleeding out as high-multiple, non-AI growth names fall out of favor with investors. Competition is also compounding the concern, with new robotic-surgery entrants like Medtronic and Johnson & Johnson pushing into the space, leading investors to question the durability of Intuitive’s near-monopoly and pricing power, which has long justified the stock’s premium valuation.
Taken together, the combination of a "sell the news" reaction to an earnings beat, a deceleration in the company’s most closely watched growth metric, an unchanged and arguably conservative guidance midpoint, and a risk-off tape pushed ISRG sharply lower in pre-market, sending shares toward the bottom of their 52-week range.
The quarter’s reported non-GAAP gross margin of 70% also benefited from a one-time $36 million pretax tariff refund, and excluding this benefit, non-GAAP gross margin would have been 68.7% — reminding investors that part of the quarter’s margin strength reflected temporary rather than recurring factors.
